Terms and conditions for 10 Class Pass

This contract is made between the client and FIT4MOM SF Valley & Northeast LA except as provided in the initial 10 day cancellation policy below. This pass allows you to attend 10 Stroller Strides classes.

AUTHORIZATION FOR PAYMENT
I authorize FIT4MOM SF Valley & Northeast LA to initiate a charge to the account entered into Front Desk for my 10 Class Pass..

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with Stroller Strides to fulfill my payment commitment and I am obligated to pay. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.

EXPIRATION POLICY
10 Class Passes expire 4 months after the date of purchase. All classes must be used within this period.

CANCELLATION POLICY
No refunds will be given for dues once it has been paid, except as outlined in the initial 10-day cancellation policy. This membership is not transferable to another person or anywhere outside the Sherman Oaks/Studio City franchise.

HOLD POLICY
No Holds are allowed on a 10 Class Pass.

INITIAL 10-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by FIT4MOM SF Valley & Northeast LA within the initial ten days. You are responsible for paying for the classes already attended at a rate of $18 per class.

AGREEMENT
By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
